Payment & Insurance

Please present your insurance cards, identification card and referral numbers to the receptionist upon your arrival. Payment is expected at the time of service for all patient balances. For more details on Medicare or insurance plans in which we are a participating provider, it is your responsibility to contact your insurance company or your primary care physician for referrals. You will be responsible for payment of any deductible amount, as well as charges not covered or disallowed by your insurance carrier. In instances of work-related injuries, it is your responsibility to verify that you have authorized workers' compensation.

Prescription Refills

You may call us during regular office hours to request medication refills. Please check with your pharmacy to verify when your prescription is ready. No prescriptions will be filled after hours or on weekends.

Medical Records

Your medical record is private and confidential and we protect it under all HIPAA laws. We will routinely forward your medical information to your referring physician. In instances of work-related injuries, records of your visit to Pinnacle will be forwarded to your workers' compensation insurance carrier. If you would like your medical records sent to anyone else, please be sure that you have signed an authorization form.

In keeping with our commitment of using leading edge technologies, Pinnacle has recently implemented an Electronic Medical Records (EMR) system. The EMR system helps to ensure the accuracy and accessibility of your records at all times. There is a charge for photocopying your records when requested for any other purpose.

Effective 10/01/2017, there is a $25 charge for all Disability and FMLA forms.  Please allow 7-10 business days for completion.
